Summary | This report displays the site number and patient ID for each patient at a site who has signed the Informed Consent Document (IDC) form. |
Charts and graphs | A bar graph displays the number of signed forms from each site. |

Column | Description |
Site Name | Full name of the site. |
Patient | Unique identifier for the patient. |
Agreed to Consent | Yes or No, indicating whether the patient agreed to sign the Informed Consent form. The table cell is blank if the patient has not yet decided whether to sign the form. |
Date Consent Signed (GMT) | Date when the patient signed the ICD form. The table cell is blank if a user did not agree to sign the ICD. |
