Field | Description | Note |

Call Center Name | Descriptive name for the Call Center. | Required. |
Call Center Phone Number | Telephone number of the Call Center. | Number where patients can contact the Call Center. You can use this number in a Call Center Script so that Call Center staff personnel can inform patients of the number. |
Max Phone Attempts | The maximum number of attempts to reach a patient allowed during a Call Center window. | Includes only phone attempts. If no total is specified, there is no limit on the number of contact attempts. |
Web Completion Window | For each regular visit in the study, the number of days and hours a survey window is open for a web-based patient. | Permanent visits, which hold forms that are cumulative throughout the study (for example, major medical events) are not associated with a Web Completion Window and do not appear in the list of visits. If the value for a visit is blank, patients are added to the Call Center queue immediately on starting the visit. |
