Initiating a patient transfer

  1. Select Patients (Patients icon. Click to navigate to the Patient List page.).

    The Patient List page appears.

  2. Select the patient you want to transfer by clicking the radio button to the left of the patient record.
  3. Click the Transfer button.

    The Patient Transfer dialog box appears. Notice that the Patient Transfer dialog box shows the Current Site and Patient ID.

  4. Review the Patient Transfer affidavit.

    Observe the color associated with Signature Status, Query Status, and Source Verification Status.

    Incomplete statuses of these items do not prevent the patient transfer, but you may want to cancel the transfer, complete these activities, and reinitiate the transfer.

  5. From the Destination Site drop-down list, select the site to which to transfer the patient.
  6. From the Reason For Transfer drop-down list, select the reason for transferring this patient.

    or

    Select Other and type the reason in the text box.

  7. Click Submit.

    If this transfer does not cause the destination site to exceed the maximum enrollment count, the transfer occurs. If site enrollment is closed, the application displays an error message. Contact someone at the destination site who can increase the patient enrollment limit.

    The application creates the transfer archive. Because the archive contains the entire patient record generated to a PDF file, archive creation can take time. During that time, do not update the patient record.

    The OutcomeLogix application displays a confirmation message that the patient was transferred successfully.

  8. Click OK.
