This report shows the measures of data-entry performance, by site and visit, by showing the number of days from:

• Date of visit to form started.
• Date of visit to form completed.
• Form completed to form source verified.
• Form completed to form locked.

Note: This calculation relies on the use of the DOV controls specified in the form design.
