The command can also fail if it failed to lock the configuration file; if there are no slots available in the UDL; if the extent to be created overlaps with an existing device on the UDL; if the first entry is being created and the device doesn't match QMCONFIG or the offset is not 0 or there is not enough space allocated to store the UDL and volume table of contents; or if a write fails on the device.
