Use the Vaccinations tile (HC_HS_SS_IMMUN_FL_GBL) to access the page to review your vaccination information.

Navigation:

Access the Vaccinations tile on the Health and Safety Dashboard, or any fluid home page to which the tile has been added.

Vaccinations tile

The Vaccinations tile displays information pertaining to the vaccination data that the employee has reported in the system. If multiple vaccination entries exist and they are in different statues, the status with the highest priority order will be displayed, as shown in this list:

  1. Action Required

  2. Submitted

The tile does not show any information if no entries with any of these statuses are found.

Use the Vaccinations - Vaccine Details page (HS_SS_IMMUN_FL) to review your vaccination information.

Click the Vaccinations tile on the Health and Safety dashboard.

Vaccine Details

This section displays a list of vaccinations reported by you or the administrator. The system stores recorded or approved vaccination data on the Health Card Page. The administrator can use this page to create, update, or delete vaccination information for the employee as well.

Select the button to report a vaccination that you had.

Action Required icon

(Action Required)

Indicates that the corresponding entry (status: More Information Requested) is pushed back by the administrator for more update.

Access the Vaccine Details Page to view approver’s comments and update the entry before submitting again.

Employee Declined icon

(Employee Declined)

Indicates that you have declined taking the associated vaccine.

Date

Displays the date the vaccine was given to you.

In case of declination, it shows the date you declined the vaccine.

Vaccine

Displays the name of the vaccine.

Status

Displays the current status of the vaccination entry. Values are:

Approved: the entry has been approved by the administrator.

Employee Declined: the entry pertains to a vaccine that has been declined by the employee.

More Information Requested: the entry has been pushed back by the administrator. The employee needs to update the entry and resubmit.

Recorded: the entry has been saved (no approval).

Request Denied: the approval of the entry has been denied by the administrator.

Submitted: the entry has been submitted and is pending approval.

If a row is added for the employee by the administrator, the status of Recorded is displayed.

Last Updated By

Displays the user who last updated the entry, which can be the employee or the administrator.

Use the Add Vaccination page (HS_SS_ADD_VACC_SCF) to add new vaccination information.

Field or Control

Description

Vaccine

Select the vaccine that you took and would like to report.

Vaccines are defined in the Immunization component.

A vaccine becomes available for selection if it matches the regulatory region of the employee and is set to be available in Self Service in its setup.

Decline Vaccine

Select to indicate that you do not wish to take the vaccine.

Use the Vaccine Details page (HS_SS_IMMUN_SCF) to enter or view vaccination information.

The setup of the selected vaccine controls the availability of fields and sections that appear on this page.

Field or Control

Description

Vaccine

Displays the vaccine selected previously.

Note: Vaccine and Date fields are read-only when you update an existing entry that is pushed back by the administrator.

Date

Enter the date you took the vaccine. Do not enter a future date.

If you choose to decline taking the selected vaccine, the system populates the current date.

Manufacturer

Select the manufacturer of the vaccine you took. This field appears if manufacturer information is specified in the setup of the selected vaccine, and information is set to be displayed in Self Service.

Dose

Select 1 if this is a single-dose vaccine or the first dose of a multi-dose series, 2 if this is the second dose, and so on. This field appears if manufacturer information is specified in the setup of the selected vaccine. It becomes editable when a manufacturer is selected.

Lot Number

Enter the lot number of the vaccine. This field appears if manufacturer information is specified in the setup of the selected vaccine. It becomes editable when a manufacturer is selected.

Location

Select the type of location where you took the vaccine. Values are:

Workplace

Location Name

Displays your work location by default and is editable, if Workplace is selected.

This field appears as a free form field if Other is selected.

Add Address

Click to enter the address of the vaccination location. This button appears if Other is selected.

Address

Displays the address that you enter manually (and editable) when you select Other as the location, or the address of the selected location name when you select Workplace as the location.

Decline Vaccine

Displays Yes if you have declined taking the vaccine.

Reason

Select the reason for declining the vaccine, if the Decline Vaccine check box is selected. Values are:

Allergic Reaction

Medical Exemption

Personal Reasons

Religious Beliefs

Comments

Enter comments about the vaccination, if any.

Next Due Date

Displays the due date of the next vaccination. It means the next first dose if the selected vaccine is a multi-dose series.

The field value appears if the schedule information is specified in the setup of the selected vaccine, and:

  • The vaccination you are reporting is either for a single-dose vaccine, or the last dose of a multi-dose series. Or,

  • The manufacturer information is not available in the vaccine setup.

The next vaccination due date is calculated as:

Next due date = vaccination date of the single-dose vaccine (or last dose of the multi-dose series) + vaccine schedule

Attachments

This section appears if attachments are enabled (or required) in the setup of the selected vaccine.

Select to upload supporting documents.

Attachments

Displays the link of the uploaded attachment, if available.

You have the option to delete attachments when you update an existing entry.

Description

Enter the description of the attachment.

Attached By and Attached On

Displays the user who uploaded the attachment and the date and time of the upload.

Acknowledgement

This section appears if acknowledgements are enabled in the setup of the selected vaccine.

Select the check box to provide your acknowledgment.

Updated By

This section appears if you are viewing an existing entry.

Approvals

This section appears if you are viewing an existing entry and approval is enabled in the setup of the selected vaccine.

Use this section to view the approval progress of the entry.

When you open an entry that is sent back to you for more update from a push notification or an email link, the system takes you to this page to make updates. Review the approver’s comments available in this section, and take the action needed before resubmitting the entry for approval.

Note: When the approver pushes back an entry, the status of the approval request is set to Terminated. This is a status used in the Approval Workflow Engine (AWE), and does not apply to the transaction of the entry that has been pushed back.

Use the View Vaccine Details page (HS_SS_IMMUNVIEW_FL) to view information of approved or denied vaccination entries.

Navigation:

Click the email link that the employee received about the vaccination entry submitted, or the push notification the employee received for the entry that has been approved or denied.
