Four Column Form Template

The DotCom Applet Form 4-Column template uses the DotComAppletForm4Col web template. It can do the following:

Define a large number of control placeholders that can span one column, two columns, or all four columns.
Map fields for up to four columns.
Display labels at start of field values.
Display validation errors at the start of the form.
Use ID 91 to add text that spans all four columns.

The following image includes the generic layout that this template uses.

At the end of the form, there is a field that spans all columns.
