Form elements and expected forms
Unless otherwise noted, all of the calculations and dates that the elements in the Forms report topic return are based on expected form counts.
A form is counted as expected if it meets any of the following criteria:
- It appears within a scheduled visit (regardless of whether the form is started).
- It appears within a started unscheduled visit.
- It is a dynamic form that has been activated.
Note: Non-started common forms are not counted as expected.
Expected forms are counted for subjects in the following states:
- Enrolled
- Randomized
- Complete
- Dropped-out
Note: For subjects that have completed or dropped-out (as indicated by the Study Completion form), only forms in started visits or dynamic started forms are counted.
The expected forms calculation assumes the following:
Only CRFs are counted. Other special CRFs (such as screening and enrollment) are not included.
- Alternate forms—One form in each visit is counted.
- Common forms—A common form is counted only once for the study, regardless of the number of visits in which it occurs.
- For enrolled or randomized subjects, common forms are considered expected.
- For complete or dropped-out subjects, only started common forms are considered expected.
- Common dynamic forms—Each activated or started instance is counted once.
- Common dynamic repeating forms—Each activated or started instance is counted once.
- Common repeating forms—Each instance is counted once.
- Dynamic forms—One activated dynamic form is counted for each visit in which it exists. Deleted (removed) form instances are not counted as expected.
- Dynamic repeating forms—Each activated or started instance is counted once.
- Regular forms—One form is counted for each visit.
- Repeating forms—For repeating forms that have not been started, one instance of the form is counted as expected. For repeating forms that have been started, each started instance counts as expected. Deleted (removed) form instances are not counted as expected.
Only subject visits and common visits are counted. Other special visits (such as screening/enrollment or monitor) are not included.
- Started visits (scheduled and unscheduled)
- Regular and repeating forms are counted as expected.
- All activated dynamic forms are counted as expected.
- All alternate forms are counted as expected.
- Unstarted scheduled visits
- For enrolled or randomized subjects:
- Regular and repeating forms are counted as expected.
- All activated dynamic forms are counted as expected.
- Started alternate forms are counted as expected.
- For complete or dropped-out subjects:
- No forms are counted as expected.
- For enrolled or randomized subjects:
- Unstarted unscheduled visits
- No forms are counted as expected.
