Forms: Form Counts by Property
Forms: Form Counts by Property report elements return totals of different types of forms. Form counts here are expected form counts.
Note: When used by themselves, these report elements return form totals multiplied by the number of subjects participating in the study. You can add other report elements to see different totals for specific subjects.

Dynamic Form |
Data type: Numeric Description: The number of expected dynamic forms. How calculated: Counts forms that are dynamic and have been activated or started. Does not count dynamic forms that have been removed. Example: A subject (who is pregnant) is participating in a study that requires the collection of additional information for subjects who are pregnant. A dynamic form is activated for the subject when entered data indicates that she is pregnant. Dynamic form count for this example = 1. Data source: Derived and stored in the Reporting and Analysis database. |
Repeating Form |
Data type: Numeric Description: The number of started, non-started, and deleted repeating forms within the study. How calculated: Counts not started repeating once. Counts each started instance of a repeating form. Example: Two subjects are participating in a study with a repeating form that records blood draws at different intervals within the study. Subject A has had one blood draw; therefore one instance of the repeating form has been started for the subject. Subject B has had two blood draws; therefore two instances of the repeating form have been started for Subject B. Repeating form count for this example =3. Data source: Derived and stored in the Reporting and Analysis database. |
Common Form |
Data type: Numeric Description: The number of possible common forms across all subjects. How calculated: Counts each common form one time, regardless of the number of visits in which the form appears. Example: A study contains a common form for Adverse Events, which appears in each visit. This form has been completed for a subject in three different visits. Common form count for this example = 1. Data source: Derived and stored in the Reporting and Analysis database. |
Alternate Form |
Data type: Numeric Description: The number of possible alternate forms across all subjects. How calculated: Counts forms that are started alternate forms. Example: An alternate form is completed for two subjects who are enrolled in the study. Alternate form count for this example=2. Data source: Derived and stored in the Reporting and Analysis database. |
Has Required Items |
Data type: Numeric Description: The number of possible expected forms across all subjects that contain required items. How calculated: Counts forms that could be expected or started and that contain required items. This calculation includes dynamic and repeating forms with required items, as follows:

Has SV Required Items |
Data type: Numeric Description: The number of possible expected forms across all subjects that contain items marked SV Required. How calculated: Counts forms that are expected or started, and that contain items marked SV Required. This calculation includes dynamic and repeating forms with items marked SV Required, as follows:
