Forms: Properties report elements return information about the properties associated with forms.

Report element

Description

Form Type

Data type: String

Description: Displays the type of form. Valid values are:

  • CRF—Identifies a form that is used in a clinical subject visit.
  • Enrollment—Identifies forms that are used in Screening and Enrollment visits.
  • Monitor RegDoc—Identifies forms that are used in Regulatory Documentation visits.
  • Monitor Visit Report—Identifies forms that are used for Visit Reports.

Use the predefined form filters with this element to limit the data on your report to a particular form type.

Data source: Derived for the report; not stored in the Reporting and Analysis database. Value decoded for display.

Is Dynamic

Data type: Y/N

Description: Specifies whether the form is dynamic. Valid values are:

  • Y—The form is dynamic.
  • N—The form is not dynamic.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database and decoded for display.

Is Repeating

Data type: Y/N

Description: Specifies whether the form is a repeating form. Valid values are:

  • Y—The form is a repeating form.
  • N—The form is not a repeating form.

A repeating form occurs multiple times in the same visit or study. For example, if a protocol calls for multiple blood draws at intervals after administering a medication, you can capture the repeating data on multiple instances of a repeating form.

Is Common

Data type: Y/N

Description: Specifies whether the form is a common form. Valid values are:

  • Y—The form is a common form.
  • N—The form is not a common form.

Is Alternate Form

Data type: Y/N

Description: Specifies whether the form is an alternate form. Valid values are:

  • Y—The form is an alternate form.
  • N—The form is not an alternate form.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database and decoded for display.

Contains SV Required Items

Data type: Y/N

Description: Specifies whether the form contains items that require Source Verification.

  • Y—The form contains SV Required items.
  • N—The form does not contain SV Required items.

    An item is considered SV Required only if it is marked SV Required.

Contains Required Items

Data type: Y/N

Description: Specifies whether the form contains items that require data entry for the form to be complete. Valid values are:

  • Y—The form contains required items.
  • N—The form does not contain required items.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database and decoded for display.

Form Order

Data type: Numeric

Description: An ordinal that specifies the position of the form within the visit. For example, 3 indicates that the form is the third form in the visit.

Design Note

Data type: String

Description: Optional text, as specified in the study design code, that is used by study designers for documentation purposes only. This note does not appear in the InForm interface.

Form Note

Data type: String

Description: Optional text, as specified in the study design code, that appears on the form.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Is SV Selected

Data type: Y/N

Description: Indicates whether the form has been selected for source verification.

  • Y—The form has been selected for source verification.
  • N—The form has not been selected for source verification.

    A form has been selected for source verification if one of the following is true:

  • The form contains one or more critical items.
  • The form is on the Critical Forms list.
  • The subject with which the form is associated is in the SV Pool.
  • The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject.
