Complete a Full Registration
Complete the following steps to perform a full registration:
- Select a facility.
Note:
The Facility value is used for MRN generation and eligibility. - Enter or update patient information, including first name, last name, and date of birth.
Note:
Is the patient emancipated and Emancipated Date are displayed if the patient is younger than the patient self-determination age. - As you create a patient, the system performs a check to ensure that the patient record you are creating does not already exist. If a potential duplicate is found, the information is displayed to you. Determine whether the potential duplicate person is different from the person you want to create.
- If you want to continue creating a new person, select X at the top of the Duplicate Profiles section.
- If you want to select the existing person, select Access Profile.
- Verify the patient information and update as needed.
- If you want to add a patient photo or identity card, see Manage a Photo or Manage an Identity Card.
- If you want to add a proxy, see Work with Proxies.
Note:
A proxy is required if the patient has not reached the patient self-determination age. If the patient is an emancipated minor, this becomes optional. - Verify contact information and update as needed.
Note:
The Contact Info section is displayed if the patient is older than the patient self-determination age or if the patient is an emancipated minor. - If notifications are enabled and you want to send the patient a text or email to complete new patient registration, select Send Self-Registration Link.
- Verify the addresses and update as needed. If you need to validate an address, see Validate an Address.
- After the patient information is populated, select Continue. The MRN is generated and assigned to the patient. If you selected the self-registration link, a text or email is sent to the patient or proxy to complete the self-registration. See Manage New Patient Registration Notification - Patient for more information.
