Update Care Team
This topic describes how to update a care team member for a patient (for example, a primary care provider or an emergency contact.
Search for Patient
Search for and select the patient to open the patient profile. See Search for Persons.
Update Care Team - Practitioner
To update a care team practitioner for a patient, access the patient information panel, then select View Care Team. See the following sections for information about adding, modifying, or removing a care team practitioner relationship.
Add a Practitioner
- Select + Add.
- From the Ongoing section, search for the practitioner. You can only select providers configured in the domain.
- Select a role, then select Save.
Modify a Practitioner
- From the Ongoing section, select the existing practitioner.
- Select Modify.
- Update the role, then select Save.
Remove a Practitioner
- From the Ongoing section, select the existing practitioner.
- Select Remove. A confirmation message is displayed.
- Select Remove.
Update Care Team - Personal
To update a personal care team member for a patient, select Care Team in the patient panel. See the appropriate section below.
Add a Care Team Member
- Select + Add. Related persons are displayed on the Related Persons list.
- If the person is found, select the person, select the role and relationship, then select Save.
- If the person is not found, select Search Person Manually.
- Search for the person by name or other search criteria.
- If the person is found, select the person, select the role and relationship, then select Save.
- If the person is not found, select + Add Person Manually.
- Select the role and relationship. Complete any additional information as needed, then select Save.
Note:
The personal care team member is added to the system as a person.
Add a Role to Care Team Member
- From the Person section, select Actions (…), then select Add Role.
- Select the roles to add, then select Save.
Modify a Care Team Member
- From the Person section, select Actions (…), then select Modify.
- Update the relationship, then select Save.
Note:
End Role and Remove Role can also be completed from the Modify action. See the End Care Team Member Role and Remove Care Team Member Role sections for more information.
End a Care Team Member Role
- From the Person section, select Actions (…), then select End Role. A message is displayed asking if you are sure you want to end the member's role.
- Select End Role. The role is ended and can be viewed in the History tab.
Remove a Care Team Member Role
- From the Person section, select Actions (…), then select Remove Role. A message is displayed asking if you are sure you want to remove the role. This action also deletes the role from the history.
- Select Remove Role. The role is removed.
Sequence Care Team Members in the Same Role
Note:
Care team members that share the same role are sequenced automatically based on entry order. The care team members that are entered most recently in the system are sequenced last, and any care team members who are not manually sequenced follow this default order.- Select a care team member.
- Select a sequence value for the care team member. This value can be primary, secondary, or tertiary.
Note:
The system can automatically resequence the order if you update a care team member’s sequence. For example, if you remove the secondary care team member, the tertiary care team member is now considered secondary.
