Import Clipboard Surveys

You can import existing HealtheLife Patient Portal Clipboard surveys to Oracle Health Patient Portal so that you do not have to re-create surveys.

Note:

Surveys and questionnaires are intended solely for patient onboarding, registration (patient intake), and gathering of information. Do not use surveys for calculations of diagnosis or for diagnostic purposes.

To import clipboard surveys:

1. Export the clipboard from HealtheLife into a .CSV file and save it to your device. Verify that the format contains the following columns:
   • Portal Question Label: The question displayed to the Patient Portal user.
   • Text Box: The options displayed to the Patient Portal user.
   • DTA ID: The ID associated with the DTA.
   • Required Field: Yes or No - whether the field must be completed to complete the survey.
   • Patient Viewable Calculation: Yes or No - whether the result is displayed to the patient.
2. From the Survey Management module, select Import Clipboard.
3. Select Choose Files in the Upload Clipboard box and select the file or drag the file to the drop area.
4. Select Import.
