Dispensation rollback actions are reflected in Oracle CRF Submit PDFs

All request types include dispensation rollback details and the associated rollback reason.

When a site user performs a dispensation rollback, the Visit History section of the Oracle CRF Submit subject PDF includes the Dispensation Rollback and Reason entries. These entries indicate that a rollback occurred and the reason for the rollback.
