5.3.1 Introduction

The A_ConditionProblem (identified/confirmable) CMET identifies observations made by certified providers for which the provider has asserted that the observation is a problem. Problems coincidently may also be recorded as diagnoses, findings, or signs and symptoms. The establishment of treatment plans and goals related to problem is not within the scope of patient problem registration.
