Fields in the Call Center History page
Field | Description | Note |
---|---|---|
Date Called (GMT) | Date and time of contact in the format selected on the User Profile page. | Dates appear in descending order. |
Visit Name | Name of the visit with which the survey is associated. |

Call Attempt | Number of attempts made to contact the patient. | Includes only contact attempts made by phone. The patient survey form status must be Submitted. |
Call Outcome | The reason for the log entry, selected from the Reason drop-down list. |

Notes | User comments about the contact. | Maximum number of characters is 400. |
User | Name of Call Center staff assigned to the patient. | The role must be Call Center Manager or Call Center Staff. |
