  Fields in the Log Call form

  Field	Description	Note
  Reason	Select a choice from the drop-down list.	Required.
  Additional Reason	Provide a reason not included in the drop-down list.
  Call Back Date	Select a date from the calendar.	The OutcomeLogix application validates the date to ensure that it is within the survey window.
  Call Back Time	Select Morning, Afternoon, or Evening.
