Viewing the archive of a transferred patient

The OutcomeLogix application generates an archive of the work completed for the patient at the originating site. The archive does not include patient-identifiable data. A table of contents provides links to the various sections of the archive and to specific visits and forms.

The archive is a snapshot of the patient record at the time of the transfer. It is a cumulative record of the patient-related activities, visits, and forms associated with the originating site. After a transfer, the originating site can see the archive only as it existed at the time of transfer.

1. Select Patients ().

   The Patient List page appears.

2. Click the Patient Transfer History tab.

   The Patient Transfer History page appears.

3. From the Search drop-down list, select your search criteria:
   • Patient List—To select from a list of patients who were transferred.
   • Transferred From—To select from a list of sites from which patients were transferred.
   • Patient ID—To search for a specific patient.
   • Transferred To—To select from a list of sites to which patients were transferred.
4. To the right of the drop-down list, click Submit ().

   The application displays the patients that match the search criteria.

5. In the Archive column of the patient whose archive you wish to view, click the View link.

   The application opens the Patient Transfer Archive Document in the Adobe Acrobat Reader application.

   You can use the navigation and viewing functions built into the version of the Acrobat Reader application you are running.

6. Close the archive by clicking the X button in the upper-right of the Acrobat Reader window.
