Patient Transfer dialog box

  Fields in the Patient Transfer window

  Field	Description	Note
  Affidavit	Study-specific guidance on the responsibility of the user regarding moving a patient from one site to another.	No acknowledgment is required.
  Patient Status
  Signature Status	Whether or not all forms have been signed.	Red text indicates attention is required.
  Query Status	Whether or not open queries exist.	Red text indicates attention is required.
  Source Verification Status	Whether or not source verification is complete.	Red text indicates attention is required.
  Patient Information
  Current Site	The site where the patient is currently enrolled.
  Patient ID	Unique identifier for the patient.
  Patient Record Transfer Information
  Destination Site	The site to which the patient is transferred.
  Reason For Transfer	Select a reason from the drop-down list, or Other to enter a reason not in the list.
  Buttons
  Submit	Initiate the transfer.
  Cancel	Close the Patient Transfer dialog box without transferring the patient.
