Entering multiple responses to an item

Some items in forms include the option to enter multiple responses; for example, if the patient has multiple diagnoses or has completed multiple procedures. If a form has been set up to allow multiple responses, an Add button appears opposite the title of the repeating section, which is outlined in green on the form.

  1. Select Patients (Patients icon. Click to navigate to the Patient List page.).

    The Patient List page appears.

  2. Click the Summary link for the patient.

    The Patient Summary page appears.

  3. Expand a visit by clicking the arrow to the left of the visit.
  4. Expand a form to show its items by clicking the arrow to the left of the form.

    Instances of repeating sections in the form appear in descending order by creation date.

  5. Click the form name.

    The form appears.

  6. Select the Add button to the right of the repeating section title.

    The part of the form containing the repeating section refreshes, the repeated section appears below the first section, and the form scrolls to the top of the new repeating section.

  7. Enter data into the repeating section.
  8. Click Submit.

    The status icon in the Form Status column of the repeating section grid changes to reflect the status of the repeating section. Data export includes the status of the repeating sections. For more information, see form and query statuses and query, review, and status icons.

    All repeating sections remain open unless you specifically collapse them.

Note: Depending on your study design, the number of times you can add a repeating section may be limited.
