Obtaining your forgotten user name or password

If you exceed the number of allowed sign in attempts, which results in a locked user account, you can reset your password and unlock your user account.

1. On the Sign In page, click the Forgot User Name or Forgot Password link.

   The Forgot User Name or Forgot Password page appears.

2. Fill in the fields.
   1. To obtain your user name, enter your email address.

      or

      To obtain your password, enter your user name.

   2. Depending on the requirements of your study, you must answer one or two reset questions.
3. Click Submit.

   If you answered the question or questions correctly, a notification appears informing you that sign in information has been sent in an email message:

   • For a forgotten user name, the email message contains your user name.
   • For a forgotten password, the email message contains a link to a webpage that prompts you to type a new password. New passwords must conform to the requirements in the password rule to which your role has been assigned.
