Forms: Form Dates

Forms: Form Dates report elements allow you to report on dates associated with form status.

Forms may be set to some states more than one time. For these states, First and Last date report elements appear in the Report Studio tree. Use these report elements to identify when a form moved from one state to another, as follows:

Use Last dates to determine the current status for a form, and the date on which the status change occurred.
Use the First and Last dates together to identify forms that were, but are no longer in a given state. You can also use these dates to identify forms that changed after being assigned a given state. For example, you can compare the Data Last Modified Date to the Last SV Complete date to see whether the form changed after it was marked SV Complete.

If a form has been in a particular state only one time, the First and Last dates for that state display the same date.

If a form has never been assigned a particular status, the columns for the First and Last dates for that state do not contain data.

Forms: Form Dates report elements

Report element	Description
Current Status Date	Data type: Date/Time Description: The date on which the form was assigned its current state. For instance, if the current state of a form is Started, this element would return the date on which the form was started. Date type: System date (GMT).
Data Last Modified Date	Data type: Date/Time Description: The date on which data was last entered or modified on the form. Date type: System date (GMT).
Form's Scheduled Visit Date	Data type: Date Description: The date on which the scheduled visit that contains the form should occur. Date type: System date (GMT).
Form's Visit Data Entry Date	Data type: Date/Time Description: The date on which data was first entered for the visit that contains the form. Date type: System date (GMT).
DOV	Data type: Date/Time Description: The date that is recorded in a standard Date of Visit control in the InForm application. Data type: Clinical Note: This report element assumes that your study was developed using a specific design for the DOV controls. Started Date	Data type: Date/Time Description: The date on which the form was started. Date type: System date (GMT).
Complete Date	Data type: Date/Time Description: The date on which the form was completed. Counts forms that are expected and meet the following criteria: Are started. Contain data for all required items. Date type: System date (GMT).
Deleted Date	Data type: Date/Time Description: The date on which the form was deleted. Date type: System date (GMT).
First Frozen Date	Data type: Date/Time Description: The date on which the status of the form was first set to Frozen. A form is frozen if it has been frozen using any of these InForm features: Freeze CRF Freeze CRB Freeze Visit This report element retrieves only those dates for forms frozen at the form or visit level. It does not retrieve a first frozen date for forms that were frozen at the case book level. Date type: System date (GMT).
Last Frozen Date	Data type: Date/Time Description: The most recent date on which the status of the form was set to Frozen. This report element retrieves only those dates for forms that were frozen at the form or visit level. It does not retrieve a last frozen date for forms that were frozen at the case book level. Date type: System date (GMT).
First SV Ready Date	Data type: Date/Time Description: The date on which the status of the form was first considered SV Ready. A form is considered SV Ready if it has a status of SV Ready, and is selected for source verification. A form has been selected for source verification if one of the following is true: The form contains one or more critical items. The form is on the Critical Forms list. The subject with which the form is associated is in the SV Pool. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject. Date type: System date (GMT).
Last SV Ready Date	Data type: Date/Time Description: The most recent date on which the status of the form was set to SV Ready. A form is considered SV Ready if it has a status of SV Ready, and is selected for source verification. A form has been selected for source verification if one of the following is true: The form contains one or more critical items. The form is on the Critical Forms list. The subject with which the form is associated is in the SV Pool. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject. Date type: System date (GMT).
First SV Partial Date	Data type: Date/Time Description: The date on which the form status was first set to SV Partial. A form is considered SV Partial if one of the following is true: The form contains unverified critical items. The form is on the Critical Forms list, and contains unverified items that are marked SV Required. The subject with which the form is associated is in the SV Pool, and the form contains unverified items that are marked SV Required. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and the form contains unverified items that are marked SV Required. Date type: System date (GMT).
Last SV Partial Date	Data type: Date/Time Description: The date on which the form was last considered SV Partial. A form is considered SV Partial if one of the following is true: The form contains unverified critical items. The form is on the Critical Forms list, and contains unverified items that are marked SV Required. The subject with which the form is associated is in the SV Pool, and the form contains unverified items that are marked SV Required. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and the form contains unverified items that are marked SV Required. Date type: System date (GMT).
First SV Complete Date	Data type: Date/Time Description: The date on which the form was first considered Verified. A form is considered Verified if one of the following is true: The form does not contain critical items, and all items on the form that are marked SV Required are verified. The form contains critical items, and all critical items on the form are verified. The form is on the Critical Forms list, and all items on the form that are marked SV Required are verified. The subject with which the form is associated is in the SV Pool, and all items marked SV Required are verified. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and all items marked SV Required are verified. If none of the items on the form require source verification, the InForm application marks the form Verified. Date type: System date (GMT).
Last SV Complete Date	Data type: Date/Time Description: The most recent date on which the form was marked Verified. A form is considered Verified if one of the following is true: The form does not contain critical items, and all items on the form that are marked SV Required are verified. The form contains critical items, and all critical items on the form are verified. The form is on the Critical Forms list, and all items on the form that are marked SV Required are verified. The subject with which the form is associated is in the SV Pool, and all items marked SV Required are verified. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and all items marked SV Required are verified. If none of the items on the form require source verification, the InForm application marks the form Verified. Date type: System date (GMT).
First Signed Date	Data type: Date/Time Description: The date on which the form was first signed. Date type: System date (GMT).
Last Signed Date	Data type: Date/Time Description: The date on which the form was last signed. Date type: System date (GMT).
First Locked Date	Data type: Date/Time Description: The date on which the form was first locked. This report element retrieves only those dates for forms that were locked at the form or visit level. It does not retrieve a first locked date for forms that were locked at the case book level. Date type: System date (GMT).
Last Locked Date	Data type: Date/Time Description: The date on which the form was last locked. This report element retrieves only those dates for forms that were locked at the form or visit level. It does not retrieve a last locked date for forms that were locked at the case book level. Date type: System date (GMT).
Review state name Date Changed	Data type: Date/Time Description: The date on which the review state for the form was last updated. Date type: System date (GMT). Notes: You create custom review states and stages for the Data Viewer using the Central Designer application. You can define as many as five custom review states, and each custom review state must include three stages. If you do not define custom review states and stages for your study, the associated report elements do not appear in the Report Studio tree. It only appears in the Form Dates folder for the Forms report topic if you select the study-specific clinical package on the Select a package page when accessing the Report Studio workspace.
