Forms: Form Dates
Forms: Form Dates report elements allow you to report on dates associated with form status.
Forms may be set to some states more than one time. For these states, First and Last date report elements appear in the Report Studio tree. Use these report elements to identify when a form moved from one state to another, as follows:
- Use Last dates to determine the current status for a form, and the date on which the status change occurred.
- Use the First and Last dates together to identify forms that were, but are no longer in a given state. You can also use these dates to identify forms that changed after being assigned a given state. For example, you can compare the Data Last Modified Date to the Last SV Complete date to see whether the form changed after it was marked SV Complete.
If a form has been in a particular state only one time, the First and Last dates for that state display the same date.
If a form has never been assigned a particular status, the columns for the First and Last dates for that state do not contain data.

Form's Scheduled Visit Date |
Data type: Date Description: The date on which the scheduled visit that contains the form should occur. Date type: System date (GMT). |

Data type: Date/Time Description: The date on which data was first entered for the visit that contains the form. Date type: System date (GMT). |

Data type: Date/Time Description: The date on which the form was started. Date type: System date (GMT). |
Complete Date |
Data type: Date/Time Description: The date on which the form was completed. Counts forms that are expected and meet the following criteria:

Deleted Date |
Data type: Date/Time Description: The date on which the form was deleted. Date type: System date (GMT). |
First Frozen Date |
Data type: Date/Time Description: The date on which the status of the form was first set to Frozen. A form is frozen if it has been frozen using any of these InForm features:
This report element retrieves only those dates for forms frozen at the form or visit level. It does not retrieve a first frozen date for forms that were frozen at the case book level. Date type: System date (GMT). |

First SV Ready Date |
Data type: Date/Time Description: The date on which the status of the form was first considered SV Ready. A form is considered SV Ready if it has a status of SV Ready, and is selected for source verification. A form has been selected for source verification if one of the following is true:

First Locked Date |
Data type: Date/Time Description: The date on which the form was first locked. This report element retrieves only those dates for forms that were locked at the form or visit level. Review state name Date Changed |
Data type: Date/Time Description: The date on which the review state for the form was last updated. Date type: System date (GMT). Notes: You create custom review states and stages for the Data Viewer using the Central Designer application. You can define as many as five custom review states, and each custom review state must include three stages. If you do not define custom review states and stages for your study, the associated report elements do not appear in the Report Studio tree. For more information, see the Central Designer InForm Design Guide. The Review state name Date report element contains study-specific clinical data. It only appears in the Form Dates folder for the Forms report topic if you select the study-specific clinical package on the Select a package page when accessing the Report Studio workspace. |
