CRF Site Performance Summary by Site

This report shows the average amount of time between study milestones, such as from visit start, to initial data entry, to form complete, and includes aging information for incomplete forms organized by site.

Tip: For the best performance, do not select any optional filters on the Selection Criteria page.

Graphs

Two bar graphs appear on the first page of the CRF Site Performance Summary by Site report. The first graph plots, for the selected sites, the median number of days from DOV to form started and the median number of days from DOV to form completed. The second graph plots the number of incomplete forms, by time period, for the selected sites.

Subtotals—By Country.
- Min—Lowest minimum value for any site in the country.
- Max—Highest maximum value for any site in the country.
- Median—No value.
Totals—Overall cycle time totals.

Column descriptions

Column	Description
Country	Data type: String Description: The country in which a site is located, as entered in the InForm Administration interface. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
Site Mnemonic	Data type: String Description: An abbreviation that identifies a site, as entered in the InForm Administration interface. Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.
DOV to Forms Started (Days): Min, Max, Median	Data type: Numeric Description: The number of days from the date entered on the DOV form to the first day on which any data entry activity (item data, comments, or queries) started. For each visit, this section displays: Minimum number of days taken by InForm users, across all forms and all subjects for each visit at every site, to start a form from the date entered in the DOV form. Maximum number of days taken by InForm users, across all forms and all subjects for each visit at every site, to start a form from the date entered in the DOV form. Median value for the number of days taken by InForm users, across all forms and all subjects for each visit at every site, to start a form from the date entered in the DOV form for all forms in the visit. DOV to Forms Completed (Days): Min, Max, Median	Data type: Numeric Description: The number of days from the date entered on the DOV form to the most recent date on which all of the required items on the form contain data. For each visit, this section displays: Minimum number of days taken by InForm users, across all forms and all subjects for each visit at every site, to complete a form from the date entered in the DOV form. Maximum number of days taken by InForm users, across all forms and all subjects for each visit at every site, to complete a form from the date entered in the DOV form. Median value for the number of days taken by InForm users, across all forms and all subjects for each visit at every site, to complete a form from the date entered in the DOV form for all forms in the visit. Forms Completed to Forms SV Complete (Days): Min, Max, Median	Data type: Numeric Description: The number of days from the earliest date on which the form was complete to the most recent date when the form was marked Verified. A form is considered Verified if one of the following is true: The form does not contain critical items, and all items on the form that are marked SV Required are verified. The form contains critical items, and all critical items on the form are verified. The form is on the Critical Forms list, and all items on the form that are marked SV Required are verified. The subject with which the form is associated is in the SV Pool, and all items marked SV Required are verified. The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and all items marked SV Required are verified. If none of the items on the form require source verification, the InForm application marks the form Verified. For each visit, this section displays: Minimum number of days, across all forms and all subjects for each visit at every site, to complete the source verification process from the date of form completion. Maximum number of days, across all forms and all subjects for each visit at every site, to complete the source verification process from the date of form completion. Forms Completed to Forms Locked (Days): Min, Max, Median	Data type: Numeric Description: The number of days from the earliest date on which the form was complete to the most recent date when the form was locked. For each visit, this section displays: Minimum number of days, across all forms and all patients for each visit at every site, to lock the form from the date of form completion. Maximum of days, across all forms and all patients for each visit at every site, to lock the form from the date of form completion. Median value of number of days, across all forms and all patients for each visit at every site, to lock the form from the date of form completion. Data source: Minimum, maximum, and median values are derived for the report, and are not stored in the Reporting and Analysis database.
Incomplete (from DOV) Forms Count (Days)	Data type: Numeric (Percentage) Description: The total count of all forms across all subjects that have been incomplete for specific ranges of calendar days after the date of visit. You specify the aging interval (in days) on the report prompt page. For example, if you specify 7, the resulting report shows the number of forms that have been incomplete for: Seven or fewer days. More than 7 but equal to or fewer than 14 days. More than 14 but equal to or fewer than 21 days. More than 21 but equal to or fewer than 28 days. More than 28 days. How calculated: This value displays as a whole number, followed by a percentage of the total incomplete form count.
