Review and Verify ID Before Your Visit

Review and verify your personal identification (ID) during the pre-visit registration process.

During theget-ready (pre-visit registration) process, scroll down to the Identification section to review and verify your identification (ID).

For any step that you do not want to complete now or you prefer to complete after you arrive, select

To review and verify your ID:

1. If you already have ID on file, review the ID select one of the following options:
   • Yes this ID is current: Verify that the ID displayed is up to date.
   • No, I need to add a new ID: Upload or use your camera to scan a new ID. See Step 4 below for more information.
   • Skip, and go to front desk on arrival: Provide this information to the front desk after you arrive to your visit.
2. If you do not yet have an ID on file, select Yes, I need to add my identification (or similarly named button) to edit your ID details as needed.
3. Select one of the following options:
   • Upload a photo of your card: Upload a photo from your device.
   • Take a photo of your card: Use your device's camera to take a photo.
4. Use your device to take a photo or upload an image of the ID. Files should be a .jpg or .png file with a maximum size of 100 MB. The patient portal uses optical character recognition (OCR) to make a copy of those details.
   The below types of ID are typically acceptable. Contact your provider for more information.

   • Driver license
   • State-issued identification card
   • Passport
5. Select Next to review your address.
6. Select Update Address to manually update your address or select Do Not Update Address to use the address that was imported from the ID.
