Review and Verify Personal Details Before Your Visit

Review and verify your identification (ID), personal information such as name and phone number, gender, and race and ethnicity during the pre-visit registration process. For any details you cannot edit, discuss these updates with the provider's office when you arrive to the visit.

During theget-ready (pre-visit registration) process, scroll down to the following sections to review and verify personal details:

• Personal information
• Gender
• Race and Ethnicity

Note:

For any step that you do not want to complete now or you prefer to complete after you arrive, select Skip, and go to front desk on arrival.

To review and verify personal details:

1. In the Personal Information section, select Edit next any personal details you want to edit. Select Save to save your changes. Personal details may include the items listed below.

   Note:

   You cannot edit some types of information, such as your first and last name, date of birth, and email address. For any changes to these types of information, discuss these updates with the provider's office when you arrive to the visit.
   • Title and suffix
   • Phone number
   • Marital status
   • Residential Address
   • Mailing Address
   When you are finished making any changes, select Yes, this personal information is correct..
2. In the Gender section, select Edit next any personal details you want to edit. Select Save to save your changes. Personal details may include the following items:
   • Birth sex
   • Gender identity
   • Pronouns
   • Sexual Orientation
   • Mailing Address
   When you are finished making any changes, select Yes, these details are correct.
3. In the Race and Ethnicity section, select Edit next any personal details you want to edit. Select Save to save your changes. Personal details may include the following items:
   • Race
   • Ethnicity
   • Preferred Language
   • Tribal Affiliation
   • Mailing Address
   When you are finished making any changes, select Yes, these details are correct.
