Review and verify the insurance and guarantor during the pre-visit registration process. A guarantor is the person financially responsible for any costs not covered by insurance.

During the get-ready (pre-visit registration) process, scroll down to the sections below to review and verify your insurance plan and guarantor details.

To review and verify the insurance and guarantor
  1. In the Insurance section, review any insurance plans listed:
    • Yes, my insurance details are correct: Verify that the insurance plans listed are still current.
    • No, I need to add a new insurance policy: Enter new insurance details. See Step 2 below for more information.
    • I'd prefer not to use insurance and pay out-of-pocket: Pay at the front desk instead of entering insurance plan details.
    • Skip, and go to front desk on arrival.: Provide this information to the front desk staff in-person when you arrive for your visit.
  2. To add a new insurance policy, select No, I need to add a new insurance policy.
    1. Select the name of the policy holder. Only people associated with the account are shown. For anyone not listed, contact the front desk when you arrive to your visit.
    2. Enter the remaining details:
      • Insurance Carrier
      • Member ID
      • Group Number
    When you are finished making any changes, select Save.
  3. In the Guarantor section, review the responsible party listed and select one of the following options:
    • Yes, this is my responsible party: Verify that the person listed is responsible for any costs not covered by insurance.
    • No, go to front desk on arrival. Provide this information to the front desk staff in-person when you arrive for your visit.
  4. Review the details and select Yes, my insurance details are correct.
