View Health Conditions in Patient Portal

In your health record, the Health Conditions page displays active and resolved health conditions documented by your provider or care team.

Note:

Depending on your organization's settings, health conditions might be alternatively labeled as Diagnoses, Health Issues, Problems, or a similar name. 

The health conditions listed include additional details if available, such as the name and date of onset.

Test results, flowsheets, and vital signs functionality are not intended to support time-critical decision-making, provide treatment, or make diagnostic recommendations. Contact your provider with any questions.

To view health conditions:

  1. If you manage multiple connected health profiles, optionally switch to a different health profile.
  2. From the navigation menu, select Immunizations.
    Alternatively, search for it by name in the Ask Oracle search box.
  3. From the My Health drop-down list, select Health Conditions. Alternatively, search for its name in the Ask Oracle search box. The list of health conditions is displayed.
  4. Select a tab:
    • Active: List of current, active health conditions.
    • Inactive: List of health conditions that are resolved or no longer active.
  5. Optionally, select Sort By Sort to sort the list by the name of the health condition or its onset date.
