View Immunizations in Patient Portal

In your health record, the Immunizations page displays current and past immunizations documented by your provider or care teams.

The immunizations listed include additional details if available, such as name, date administered, how many immunizations for that particular immunization type, and any history of similar immunizations.
To view immunizations:
  1. If you manage multiple connected health profiles, optionally switch to a different health profile.
  2. From the navigation menu, select Immunizations.
    Alternatively, search for it by name in the Ask Oracle search box.
  3. Optionally select Sort By Optionally, select Sort By Sort to sort the list of immunizations by name or administration date.
  4. Select an immunization to view its details (as available):
    • Date received
    • Immunization product name
    • Lot number
    • Manufacturer
  5. Optionally, select Print to print your list of immunizations.

To update your list of immunizations, meet with your provider to discuss and make any changes.
