Assembling the Categories Within the Questionnaire
Use the Assemble Questionnaire page to assemble the categories within the questionnaire. You can update the questionnaire, if required.

To assemble categories within the questionnaire:
1. Select Assemble Questionnaire from the main menu.
2. Select the questionnaire you created within which you want to assemble the categories.
3. Click the New button to assemble the new categories.
4. Select the first category you want to display on the questionnaire in the Category Name field.
In our example, this is Overall Assessment.
Note: Do not enter a category in the Parent Category field unless you are entering minor headings (sub-categories ). If you want to enter minor headings, see Entering Minor Headings.
5. Enter 1 in the Parent Display Order field if you want this category to be the first one displayed on the questionnaire.
6. Enter 1 in the Category Display Order field if you want this category to be the first one displayed on the questionnaire.
7. Leave the Display Category Header set to Yes if you want the category to be displayed on the questionnaire.
8. When you have assembled up to three categories, click the Save button.
To enter further categories:
1. Click the New button again after assembling the previous three categories.
2. Enter details of further categories in the same way as before.
3. Increment the number in the Parent Display Order field by one to indicate the order in which you want the category to display on the questionnaire.
For example, enter 2 in the Parent Display Order field if you want the category to display second, enter 3 if you want it to display third, and so on. Enter 1 in the Category Display Order field as you want this category to be the first displayed for the second category.
In our example, select Areas for Development and Improvement (in the Category Name field), enter 2 (in the Parent Display Order field), enter 1 (in the Category Display Order field) and Leave the Display Category Header set to Yes.
4. Click the Save button when you have entered all the categories you want.
5. Click the Layout button to see all the categories for this questionnaire.
To enter sub-categories:
1. To enter minor headings (sub-categories), move to the next blank line and select the parent category under which you want to display the heading in the Parent Category Name field.
In our example, select Areas for Development and Improvement (because this is the category under which you want to display Strengths and Improvement Areas and Development Actions Resulting from Performance Review).
2. Select the sub-category you want to display beneath the parent category in the Category Name field
In our example, select Strengths and Improvement Areas.
3. Leave the Parent Display Order field blank.
4. Enter the order in which this sub-category displays under the parent category.
In our example, enter 2 in the Category Display Order field. This is because the parent category (Areas for Development and Improvement) is the main heading and Strengths and Improvement Areas is second.
5. Leave the Display Category Header set to Yes.
If you have other sub-categories to include in the questionnaire, continue to enter them as described above, but continue to leave the Parent Display Order field blank, and increment the Category Display Order field by one.
In our example, select Areas for Development and Improvement in the Parent Category Name field then select Development Actions Resulting from Performance Review in the Category Name field. Leave the Parent Display Order field blank and enter 3 in the Category Display Order field. Leave the Display Category Header set to Yes.
6. Click the Save button when you have entered all the categories you want.
7. Click the Layout button to see all the categories for this questionnaire.
