Patient Profile page

The Patient Profile page contains both standard and study-specific information about a patient. The fields in the following table are examples of standard information usually included in the patient profile if the Call Center is enabled. The Patient Profile page for your study might contain all or none of these fields as well as additional study-specific fields.

If the study is designed to blind Personal/Protected Health Information to specific users, and your user account is not permitted to view Personal/Protected Health Information, you cannot view the Patient Profile page.

• Viewing and editing a patient profile.
• Locking and unlocking a patient.
• Viewing the audit trail for a patient.
• Managing patients and sites.

  Fields in the Patient Profile page

  Call Center fields
  Preferred Method of Contact	Survey completion mode: Web, Phone, or Paper.	Required.
  First Name	First name of the patient.	Required.
  Middle Name	Middle name of the patient.
  Last Name	Last name of the patient.	Required.
  Language	Language of the patient. After you make a change and submit, all forms that have been translated to the selected language appear in the selected language.	Required.
  Email	Email address for notifications. This is also the user name for the patient.	Required if the Preferred Method of Contact is Web. Maximum length: 40 characters. The ending must be standard for an email address, such as .com, .org, or .edu.
  Phone	Phone number to contact the patient.	Required if the Preferred Method of Contact is Web or Phone.
  Confirm Phone	Confirmation of phone number to contact the patient.	Required if the Preferred Method of Contact is Web or Phone.
  Time Zone	Time zone in which the patient resides. After you make a change and submit, time values use the selected time zone.	Required if the Preferred Method of Contact is Web or Phone.
  Preferred Time of Contact	Set a contact range by moving the sliding controls to the start and end times of the preferred range.	Required if the Preferred Method of Contact is Web or Phone.
  Address1	Mailing address of the patient.
  Address2	Mailing address of the patient.
  City	City where the patient is located.
  Zip Code	ZIP code where the patient is located.
  State	State where the patient is located.
  Country	Country of residence.
  Patient Profile Core Data fields These fields appear if ShowPatientProfileCoreData is set to Y. For more information, see Configure Study Settings page.
  Site ID	Unique identifier of the site to which the patient is assigned.
  Patient ID	Unique identifier assigned to the patient.
  Last 4 of the SSN	The last four digits of the patient Social Security Number.
  Legally Authorized Representative (optional)	Name of an individual legally responsible for the patient.
  Legally Authorized Representative telephone number (optional)	Telephone number of the representative.
  Email Address	Email address for notifications. This is also the user name.
  User Name	Email address of the patient.	Read-only.
  Memorable Date	Month and year of significance to the patient. Used to authenticate patient identity by Help Desk personnel.
  Site Name Site Number Organization	The name of the site, its number, and the name of the organization of which the site is a part.	Read-only.
