CRF Status Detail by Visit and Site

This report shows the number of forms in each possible status, organized by visit across sites. Use the report to help focus on cleaning specific visits.

Tip Tip: For the best performance, do not select any optional filters on the Selection Criteria page.

Description

Country

Data type: String

Description: The country in which a site is located, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Visit Mnemonic

Data type: String

Description: An abbreviation of the visit title, as specified in the study design. For CRF visits, this title appears at the top of a collection of forms in the visit timeline and may not be a unique identifier.

Site Mnemonic

Data type: String

Description: An abbreviation that identifies a site, as entered in the InForm Administration interface.

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database.

Subject Number

Data type: String

Description: The number that identifies the subject. This number can also be changed post-enrollment, if configured in the study.

Note: Subject number might not be unique at your site.

DOV

Data type: Date/Time

Description: The date that is recorded in a standard Date of Visit control in the InForm application.

Note: This report element assumes that your study was developed using a specific design for the DOV controls.

Form Count (% of Expected): Expected

Data type: Numeric

Description: The number of expected forms in the study.

How calculated: This number reflects the number of expected forms for the site based on the option selected for the Expected forms for filter on the report prompt page. For more information, see Form elements and expected forms.

Data source: Derived for the report; not stored in the Reporting and Analysis database.

Form Count (% of Expected): Incomplete

Data type: Numeric (Percentage)

Description: The number of expected forms that are started but incomplete, and the percentage of expected forms that are incomplete.

How calculated: For each site, calculates the following:

[incomplete forms count] = [started forms count] - [completed forms count]

[incomplete, % of expected] = [incomplete forms count] * 100 / [expected forms count]

Data source: Derived for the report; not stored in the Reporting and Analysis database.

Form Count (% of Expected): Completed

Data type: Numeric (Percentage)

Description: The number of completed forms, and the percentage of expected forms that are complete in the InForm application.

How calculated: Counts forms that are expected and meet the following criteria:

  • Are started.
  • Contain data for all required items.
  • For a repeating form, the form is not in a deleted state.
  • For an itemset form, the form is complete even if all the instances are deleted.

This value displays as a whole number and as a percentage of the total number of expected forms that are complete:

Form Count (% of Expected): Frozen

Data type: Numeric (Percentage)

Description: The number of frozen forms, and the percentage of expected forms that are currently marked frozen in the InForm application.

How calculated: Counts expected forms that have been frozen using any of these InForm features:

  • Freeze CRF
  • Freeze CRB
  • Freeze Visit

This value displays as a whole number (number of frozen forms), followed by the percentage of expected forms that are currently marked frozen:

Form Count (% of Expected): SV Partial

Data type: Numeric (Percentage)

Description: The number of forms marked SV Partial, and the percentage of expected forms where source verification is not complete.

How calculated: Counts partially verified forms.

A form is considered SV Partial if one of the following is true:

  • The form contains unverified critical items.
  • The form is on the Critical Forms list, and contains unverified items that are marked SV Required.
  • The subject with which the form is associated is in the SV Pool, and the form contains unverified items that are marked SV Required.
  • The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and the form contains unverified items that are marked SV Required.

This value is displayed as a whole number and as a percentage of the total number of expected forms that are marked SV Partial:

[SV Partial, % expected] = [SV Partial forms count] * 100 / [expected forms count]

Data source: Replicated from the InForm transactional database to the Reporting and Analysis database. Percentage is derived for display.

Form Count (% of Expected): SV Completed

Data type: Numeric (Percentage)

Description: The number of verified forms, and the percentage of expected forms where source verification is complete.

How calculated: Counts forms marked Verified.

A form is considered Verified if one of the following is true:

  • The form does not contain critical items, and all items on the form that are marked SV Required are verified.
  • The form contains critical items, and all critical items on the form are verified.
  • The form is on the Critical Forms list, and all items on the form that are marked SV Required are verified.
  • The subject with which the form is associated is in the SV Pool, and all items marked SV Required are verified.
  • The subject with which the form is associated is not in the SV Pool, but an item on the form is marked critical for that subject, and all items marked SV Required are verified.

If none of the items on the form require source verification, the InForm application marks the form Verified.

This value displays as a whole number and as a percentage of the total number of expected forms that are marked Verified.

Form Count (% of Expected): Signed

Data type: Numeric (Percentage)

Description: The number of signed forms and the percentage of expected forms that have been signed in the InForm application.

How calculated: Counts forms that have been signed. This value displays as a whole number and as a percentage of the total number of expected forms that are signed.

Form Count (% of Expected): Locked

Data type: Numeric (Percentage)

Description: The number of locked forms, and the percentage of expected forms that are locked.

How calculated: Counts forms for which any of the following conditions is true:

  • The form has been locked.
  • The form is contained within a case book that has been locked.
  • The form is contained within a visit that has been locked.

This value displays as a whole number and as a percentage:

[locked, % of expected] = [locked forms count] * 100 / [expected forms count]

Form Counts (% of Expected): Forms with Comments

Data type: Numeric (Percentage)

Description: The number of expected forms with form-level comments, and the percentage of expected forms that have form-level comments.

How calculated: Counts forms that contain form-level comments. Does not count forms with item-level comments only. This value displays as a whole number and a percentage of the total number of expected forms that have comments:

Form Counts (% of Expected): Forms with Queries

Data type: Numeric (Percentage)

Description: The number of forms with open queries, and the percentage of expected forms that contain open queries.

How calculated: Counts forms that contain an item with an open query. This value displays as a whole number and a percentage of the total number of expected forms that have open queries:
