Review and Sign Consent Forms Before Your Visit

Review and sign any required consent forms during the pre-visit registration process.

During theget-ready (pre-visit registration) process, scroll down to the Consent Forms section to review your and sign consent forms.
To review and sign consent forms:
  1. Select a form to display it. If you are using a web browser, the form may be displayed in a new tab.
  2. Review the form and scroll to each section.
  3. Select any checkboxes and complete any fields as required.
  4. At the bottom of the form, select the Signature box to display the signature window.
    The signature window includes the following items:
    • Please Sign Below: Use your finger or mouse to sign in the signature box
    • Clear: Remove the content from the Signature box so you can try again.
    • Decline: Indicates that you do not want to sign this form. In place of a signature, the text DECLINED is displayed in the signature box.
    • Cancel: Close the signature window without making any changes.
    • Reset: Closes the signature window and resets the signature box to be blank.
    • Done: Close the window and apply the signature to the consent form.
  5. To add your signature, use your finger or mouse to sign in the signature box and then select Done.
    Alternatively, select Decline to indicate that you do not want to sign this form.
  6. Verify that the date and time is automatically displayed in the Date/Time box.
  7. Use the next (>) and previous (<) buttons to navigate between forms.
  8. Review, complete, and sign each form as needed.
  9. Select Finish to close the consent forms and return to the get-ready workflow.
