Complete Questionnaires in Patient Portal

Questionnaires (also called clipboards, e-clipboards, registration forms, or surveys) may be assigned to you by your healthcare organization. Complete these forms before your scheduled visit.

Note:

Questionnaires are not intended to be used for calculations or for diagnostic or therapeutic purposes. Questionnaires are intended only for patient onboarding, registration (patient intake), and information gathering.
The list of questionnaires is determined by your provider and visit type. Depending on your provider and visit type, the following types of questionnaires may be included:
  • Health history questionnaires:
    • Allergies
    • Medications
    • Procedures
    • Health conditions
    • Immunizations
    • Social history
    • Family history
  • Other custom questionnaires assigned by your provider

Note:

Depending on your organization's settings, health conditions might be alternatively labeled as Diagnoses, Health Issues, Problems, or a similar name. 
  1. Access questionnaires in one of the following ways:.
    • From the navigation menu, select Questionnaires. Select Begin next to the questionnaire you want to complete.

      Note:

      If you exit a questionnaire before you submit it, the button displays the label Resume instead.
    • During theget-ready (pre-visit registration) process, scroll down to the Health History section to complete the questionnaires assigned to you.
    • Search for a questionnaire form by name in the Ask Oracle search box.
  2. Depending on the type of form, the following actions may be available to you:
    • To edit any detail (available on applicable questionnaires only), select Edit and make your changes. Select Save when you are done.
    • To add new details (available on applicable questionnaires only), scroll to the bottom of that form and select No, I need to add a new [item] (or similarly named button) to add new details.
  3. If you are viewing the form in the Questionnaires section of the website, select Submit to submit your answers to your provider.

    Note:

    You can submit questionnaires and forms only once. After you submit them, you are unable to view or update the information. Contact the provider's office when you arrive to make any additional changes.
    Otherwise, scroll down to the next section of the pre-visit registration questionnaires to continue completing additional sections. For custom forms, select Save and Continue to save your progress.

    Tip:

    Responses are automatically saved in progress until the questionnaires are submitted.
