7 Health Profile

Depending on your organization and settings, the Health Profile section in Patient Portal might include options for you to view and manage your health, such as allergies, immunizations, and health conditions.

• View Medications in Patient Portal
  In your health record, the Medications page displays active and past medications documented by your provider or care team.
• View Test Results in Patient Portal
  In your health record, the Test Results page displays test results documented by your provider or care team, for example, blood tests and cholesterol levels.
• View Visits in Patient Portal
  In your health record, the Visits page displays a list of your upcoming and past provider visits.
• View Vital Signs in Patient Portal
  In your health record, the Vital Signs page displays the vital signs and body measurements documented by your provider or care team. Vital signs can include information such as height, weight, blood pressure, and pulse. 
• View Growth Charts in Patient Portal
  If your organization provides you with access to a growth chart, you can access this feature from the navigation list (Home) menu.
• View Health Conditions in Patient Portal
  In your health record, the Health Conditions page displays active and resolved health conditions documented by your provider or care team.
• View Allergies in Patient Portal
  In your health record, the Allergies page displays current and past allergies documented by your provider or care team.
• View Procedures and Surgeries in Patient Portal
  In your health record, the Procedures page displays current and past procedures documented by your provider or care team.
• View Family History in Patient Portal
  In your health record, the Family History page displays social health history documented by your provider or care team.
• View Social History in Patient Portal
  In your health record, the Social History page displays social health history documented by your provider or care team.
• View Immunizations in Patient Portal
  In your health record, the Immunizations page displays current and past immunizations documented by your provider or care teams.
• Complete Questionnaires in Patient Portal
  Questionnaires (also called clipboards, e-clipboards, registration forms, or surveys) may be assigned to you by your healthcare organization. Complete these forms before your scheduled visit.
• Download or Share a Health Record from Patient Portal
  You can easily download or share all or parts of your health record.
• View Documents in Patient Portal
  In your health record, the Documents page displays clinical notes documented by your provider or care team.
