5 My Health

Depending on your organization and settings, your Patient Portal might include options for you to view and manage your health, such as allergies, immunizations, and health conditions. You can also download the details to a PDF file and share the data.

Note:

Health information is displayed for the currently selected profile only. If you manage profiles for others, select the Switch Profile menu (your name) at the top of the page to switch another profile. See Switch Profile Menu for more information.

• My Health Dashboard
  The My Health dashboard allows you to view all of your health record information.
• Confidentiality and Sensitive Health Record Details
  Your health record may include certain information marked as sensitive.
• Access a Health Record
  Patient portal users can access their health record through My Health. You can view, download, and share the health record for yourself or anyone whose care you manage.
• Download a Health Record
  As a Patient Portal user, you can view and download all or parts of your own health record or the health record of a person for whom you act as an authorized representative (proxy user).
• Send a Health Record
  You can share all or part of a health record with a provider or another person through email. This Consolidated Clinical Document Architecture (C-CDA) record is usually sent to physicians or specialists at a different hospital to give them a complete understanding of current health and health history. 
• View Allergies
  In your health record, the Allergies page displays current and past allergies documented by your provider or care team.
• View Documents
  In your health record, the Documents page displays clinical notes documented by your provider or care team.
• View Family History
  In your health record, the Family History page displays social health history documented by your provider or care team.
• View Health Conditions
  In your health record, the Health Conditions page displays active and resolved health conditions documented by your provider or care team.
• View Immunizations
  In your health record, the Immunizations page displays current and past immunizations documented by your provider or care teams.
• Medications
  Patient Portal allows you to view your current and past medications. Depending on your organization and settings, your Patient Portal might also allow you to submit requests to refill and renew prescriptions.
• View Procedures
  In your health record, the Procedures page displays current and past procedures documented by your provider or care team.
• View Social History
  In your health record, the Social History page displays social health history documented by your provider or care team.
• View Test Results
  In your health record, the Results page displays current and past test results documented by your provider or care team, for example, blood tests and cholesterol levels.
• View Vital Signs
  In your health record, the Vital Signs page displays the vital signs and body measurements documented by your provider or care team. Vital signs can include information such as height, weight, blood pressure, and pulse.
