View Growth Charts in Patient Portal

If your organization provides you with access to a growth chart, you can access this feature from the navigation list (Home) menu.

Note:

Test results, flowsheets, and vital signs functionality are not intended to support time-critical decision-making, provide treatment, or make diagnostic recommendations. Contact your provider with any questions.
Each growth chart allows you to view the documented measurement as reference lines on a graph.
You can view the following types of charts (when applicable):
  • Weight-for-age: 0 to 36 Months
  • Length-for-age: 0 to 36 Months
  • Weight-for-recumbent length: 0 to 36 months
  • Head circumference-for-age: 0 to 36 months
  • Weight-for-height: 2-5 Years
  • Weight-for-age: 2 to 20 Years
  • Height-for-age: 2 to 20 Years
  • BMI-for-age: 2 to 20
  • Years Extended BMI: 2-20 Years

Note:

The system only graphs measurements of the same type (height versus weight) and in the same age range. For example, any height that is displayed on one growth chart for height by age, is unavailable for viewing on another height-by-age growth chart if the chart is in the same age range.
The system displays the details below in a tooltip for each observation.
  • Patient age
  • Observation value
  • Percentile value
  • Z-score
  • Growth velocity
  • Date and time the observation was documented

Note:

Growth charts use L, M, and S data as described on the CDC website to perform calculations for percentile and z-scores. See CDC Growth Charts Data Files at the CDC.gov website for information.

To view growth charts:

  1. If you manage multiple connected health profiles, optionally switch to a different health profile.
  2. From the navigation menu, select Growth Charts.
  3. Select any growth chart to view more information about it.
