View Social History in Patient Portal

In your health record, the Social History page displays social health history documented by your provider or care team.

To view social history:

1. If you manage multiple connected health profiles, optionally switch to a different health profile.
2. From the navigation menu, select Social History.
3. Select a category to display its information.
