The Condition Problem RMIM represents Observations made on a patient by certified providers for which the provider has asserted that the observation is a problem. Problems may coincidently also be recorded as diagnoses, findings, or signs and symptoms. The establishment of treatment plans and goals related to problems is not within the scope of patient problem registration.
