The Notifiable Condition (Case Report) RMIM addresses the contents of the report of a confirmed or suspected case of a notifiable condition that is generated by a provider or a public heath entity, and transmitted to a public health entity. It provides a means to pass information about a notifiable condition (case), about the investigation into that case, about the person or other entity who is the subject of the case, other notifications for the same case, and clinical information related to the case.
It typically describes an incident of disease, whether infectious or non-infectious. It may also reflect an incident of another type, such as an automobile or a violent crime that is being tracked for public health purposes. In the United States, identifying data for affected purpose is commonly removed when cases are reported from a State Health Department to the Federal government (for example, Centers for Disease Control and Prevention). There are other circumstances, for example, reporting from a local authority or provider to a state health department, or reporting between states in which identifying information is included.
