Issuing a manual query
You can issue a manual query on the patient, visit, form, or item level as long as the patient, visit, form, or item is unlocked. You cannot issue queries on ePRO forms.
A sponsor user, typically the clinical data manager or medical monitor, performs this action. For more information, see Query types.
Issuing a query from the Patient Summary page:
- Select Patients (
).The Patient List page appears.
- Click the Summary link for the patient.
The Patient Summary page appears.
- Expand a visit by clicking the arrow next to the visit name.
The forms in the visit appear.
- Expand a form by clicking the arrow next to the form name.
The items on the form appear.
- Select the checkbox for one or more items.
- Click the Create Query icon (
). - In the dialog box, select the reason for the query from the Query Selection drop-down list.
If you select Other from the drop-down list, you must enter the reason in the Other Text box.
- To create a candidate query, select the Issue as a Candidate Query? checkbox.
The query is visible to sponsor personnel only.
- Click Submit.
An Open query status icon (
) appears in the Status column next to the items, the form, the visit, and the patient. If you created a candidate query, sponsor users see a Candidate query status icon (
).
Issuing a query from a form:
- Open the form. For more information, see Entering and editing data on a form.
Note: The application displays a message that you have read-only privileges. This does not affect your ability to issue queries on the form or item.
- Click the Create Query icon (
).A dialog box appears.
- To create a candidate query, click the Issue as a candidate query? checkbox.
- To issue a query on the form as a whole, leave the Issue as an Item query? checkbox unchecked.
or
To issue a query on an item within the form:
- Click the Issue as an Item query? checkbox.
- From the Item Name drop-down list, select the item.
- Select the reason for the query from the Query Selection drop-down list.
If you select Other from the drop-down list, you must enter the reason in the Other Text field.
- Click Submit.
If you created a query for the form, an Open query status icon appears after the Status field at the top of the form and next to the form name.
If you created a query for a particular item, an Open query status icon appears after the Status field and to the right of the item.
