Access a Health Record

Patient portal users can access their health record through My Health. You can view, download, and share the health record for yourself or anyone whose care you manage.

Note:

Your health record might contain sensitive personal health information. Be careful where you save and store your personal details, and share it with only people you trust.
Your electronic health record (EHR) is a secure, digital version of your patient chart. You can download it to your device or you can send it to other providers. Your EHR includes only information that has been reviewed and added or approved by your provider.

Note:

When you sign in to the patient portal and access My Health, your personal health record (PHR) is displayed, divided into separate pages for each category (allergies, test results, and so on). This PHR is a local version of your health record and can include additional personal details that you add, such as diary entries. This information is not part of your electronic health record (EHR) until it is reviewed and added by your provider. Contact your provider if you have questions about your patient chart or if your want to make any changes to it. Depending on your organization and account settings, some health record details might not be displayed immediately or might be delayed.
To access a health record:
  1. Sign in to Patient Portal. If you manage the account of others, select your name to switch the profile to the appropriate person.

    Note:

    The health record you download includes only information for the currently selected profile. If you are an authorized representative (proxy user) for other profiles, see Switch Profile Menu for information about how to switch to a different profile.
  2. From the Ask Oracle menuAsk Oracle, select My Health. Alternatively, select the My Health dashboard tab at the bottom of the page. Your health record is displayed.
  3. From the My Health menu at the top of the page, select the page you want to open:
    • Allergies
    • Documents
    • Health Conditions
    • Family History
    • Immunizations
    • Test Results
    • Medications
    • Procedures
    • Social History
    • Vital Signs

    Note:

    • Depending on your organization's settings, health conditions might be alternatively labeled as Diagnoses, Health Issues, Problems, or a similar name.
    • Depending on your organization and account settings, some health record details might not be displayed immediately or might be delayed.
  4. Optionally, select Download Record or Share Record to download or share your health record in a Consolidated Clinical Document Architecture (C-CDA) format. See the following topics to learn more about the file types and the information that is included:
