Send a Health Record

You can share all or part of a health record with a provider or another person through email. This Consolidated Clinical Document Architecture (C-CDA) record is usually sent to physicians or specialists at a different hospital to give them a complete understanding of current health and health history.

To share a health record:
  1. Sign in to Patient Portal. If you manage the account of others, select your name to switch the profile to the person for whom you're wanting to download the health record.

    Note:

    The health record you download includes only information for the currently selected profile. If you are an authorized representative (proxy user) for other profiles, see Switch Profile Menu for information about how to switch to a different profile.
  2. From the Ask Oracle menuAsk Oracle, select My Health. Alternatively, select the My Health dashboard tab at the bottom of the page.
  3. Select Share Health Record.
  4. Enter or select the email address where you want to send your record. Records sent to email addresses that end in direct.com use enhanced security. Other types of email addresses use standard security.

    Note:

    • This file might contain sensitive personal health information. Be careful when sending your personal details, and share it with only people you trust.
    • Contact your provider to ask whether they have a Direct email address.

      Note:

      Direct email addresses are different from standard email addresses and are used by providers to securely send and receive encrypted health information.
  5. Select Continue. The Select Encounters (or similarly named) page is displayed.
  6. Select the file size:
    • Entire Record: Send your entire record.
    • Partial Record: Select the visits (also called encounters) you want to send.
      1. Select Date Range
      2. Select or enter the start date and end date.
      3. Select Apply. The list of encounters is updated to a limited number based on the date range that you selected. 
      4. Select the checkboxes next to each encounter you want to include.
  7. Select Continue. The New Message page is displayed.
  8. Enter a brief message to include with the file.
  9. Select Return Contact Method to enter a phone number and email address where you want the recipient to respond.
  10. Optionally, select Edit to make any changes or select a different date range.
  11. Select Send. The Today dashboard opens and a success message is displayed at the bottom of the page. If individual encounters were selected, an XML and PDF file are  sent for each encounter. If the entire record was selected, an XML and PDF file are sent for the entire record.
