Download a Health Record
As a Patient Portal user, you can view and download all or parts of your own health record or the health record of a person for whom you act as an authorized representative (proxy user).
Note:
This file might contain sensitive personal health information. Be careful where you save and store your personal details, and share it with only people you trust.Health record downloads include selected visits (also called encounters). As part of the download process you can select whether to download all visits (your entire health record) or only selected visits (a partial health record). If individual visits are selected, an XML and PDF file are downloaded for each visit. If the entire record was selected, a XML and PDF file are downloaded for the entire record.
- PDF: A human-readable file that you can save, print, or view on most computers and smartphones.
- XML: A machine-readable Consolidated Clinical Document Architecture (C-CDA) file that you can optionally upload to another organization's health record system or share with a physician who is requesting this format.
Note:
C-CDA is a health record document standard used by U.S. healthcare organizations for sharing electronic health records. The XML format is commonly used by physicians or specialists to share health record information between hospitals and to give them a detailed understanding of current health and health history.
Each file (regardless of format) includes the following information:
Downloaded files do not include any personal health record (PHR) items such as diary entries or self-entered conditions (that have not yet been reviewed or added to your electronic health record by your provider). - Continuity of Care Document (CCD) of clinical notes (if present) and visit summaries
- Discharge summaries (if present)
- Referral notes (if present)
- Transition of care notes (if present)
Note:
CCDs are generated by the system when a patient portal user requests them. Discharge summaries, referral notes, and transition of care notes are generated by the system only when the provider requests them. For this reason, these other documents are included in your download only if the provider has previously requested this information.
