Download a Health Record

As a Patient Portal user, you can view and download all or parts of your own health record or the health record of a person for whom you act as an authorized representative (proxy user).

Note:

This file might contain sensitive personal health information. Be careful where you save and store your personal details, and share it with only people you trust.
Health record downloads include selected visits (also called encounters). As part of the download process you can select whether to download all visits (your entire health record) or only selected visits (a partial health record). If individual visits are selected, an XML and PDF file are downloaded for each visit. If the entire record was selected, a XML and PDF file are downloaded for the entire record.
  • PDF: A human-readable file that you can save, print, or view on most computers and smartphones.
  • XML: A machine-readable Consolidated Clinical Document Architecture (C-CDA) file that you can optionally upload to another organization's health record system or share with a physician who is requesting this format.

    Note:

    C-CDA is a health record document standard used by U.S. healthcare organizations for sharing electronic health records. The XML format is commonly used by physicians or specialists to share health record information between hospitals and to give them a detailed understanding of current health and health history.
Each file (regardless of format) includes the following information:
  • Continuity of Care Document (CCD) of clinical notes (if present) and visit summaries
  • Discharge summaries (if present)
  • Referral notes (if present)
  • Transition of care notes (if present)

Note:

CCDs are generated by the system when a patient portal user requests them. Discharge summaries, referral notes, and transition of care notes are generated by the system only when the provider requests them. For this reason, these other documents are included in your download only if the provider has previously requested this information. Contact your provider with any questions about these documents.
Downloaded files do not include any personal health record (PHR) items such as diary entries or self-entered conditions (that have not yet been reviewed or added to your electronic health record by your provider). To download a health record:
  1. Sign in to Patient Portal. If you manage the account of others, select your name to switch the profile to the person for whom you're wanting to download the health record.

    Note:

    The health record you download includes only information for the currently selected profile. If you are an authorized representative (proxy user) for other profiles, see Switch Profile Menu for information about how to switch to a different profile.
  2. From the Ask Oracle menu Ask Oracle, select My Health. Alternatively, select the My Health dashboard tab at the bottom of the page.
  3. Select Download Record.
  4. Select the file size:
    1. Entire Record: Download your entire record.
    2. Partial Record: Select which visits (also called encounters) you want to download.
      1. Select Date Range
      2. Select or enter the start date and end date.
      3. Select Apply. The list of encounters is updated to a limited number based on the date range you selected. 
    3. Select the checkboxes next to each encounter you want to include.
  5. Select Download. If individual encounters were selected, an XML and PDF file are downloaded for each encounter. If the entire record was selected, a XML and PDF file are downloaded for the entire record.
