Deactivate a Patient Portal Account

Deleting an account deletes the identity and access management associated with your patient portal account. This action closes your Patient Portal account, but it does not delete your provider's electronic health record. Contact your provider's office to permanently remove your electronic health record.

Note:

Deleting your account does not remove your electronic health records. To delete health records, contact your provider.
To delete your Patient Portal account and all associated health profiles:
  1. From the navigation menu, select Settings. The Settings menu is displayed.
  2. Select Deactivate Account.
  3. Select Deactivate to confirm that you want to deactivate your account.

An error message will be displayed if you try to sign in to the account you deactivated.

Contact your provider to regain access to your electronic health record. You cannot re-gain any settings or personal information you previously added.
