2 Accounts, Profiles, and Settings
To start using Patient Portal, you must create an account and enroll. After you create an account, you can manage your profile, update your account settings, and connect additional profiles as needed.

- Activate a New Account and Enroll in the Patient Portal
To access Patient Portal, you need an account. The enrollment process to create a new account might look slightly different to you, depending on your healthcare organization. - Sign In to Patient Portal
After you enroll, you can sign in to Patient Portal and view your health profile. - Sign Out of Patient Portal
Sign out when you are done using the portal to protect the security of your account and personal details. Signing out is especially important if you use a shared computer. - Deactivate a Patient Portal Account
Deleting an account deletes the identity and access management associated with your patient portal account. This action closes your Patient Portal account, but it does not delete your provider's electronic health record. Contact your provider's office to permanently remove your electronic health record. - View the Patient Portal Terms of Use and Privacy Policy
You can access the Terms of Use and the Privacy Policy for Patient Portal at any time. Additional legal documents, such as end-user license agreements may be available. This information can change depending on the requirements of your organization and country. - Change a Password
You can change your password anytime from the sign-in page or from the Sign-In and Security settings. - Recover a Forgotten Password
If you forgot your password, you can reset from the sign-in page. - Unlock a Patient Portal Account
Your account might become temporarily locked if you try to sign in unsuccessfully five times in a row. - Switch Health Profiles as an Authorized Representative or Caregiver in Patient Portal
If you are an authorized representative (proxy user) who has access to other connected profiles, you can switch to those other profiles to view the other person's health record and manage their care, for example, schedule appointments, view vaccine history, request renewals on prescriptions, and more. - Identity and Confidentiality in Patient Portal
Authorized representatives (also known as a proxy users) are commonly the caregiver or legal guardian of someone else, for example, a child or elderly parent. - Manage Settings and Preferences in Patient Portal
You can update your account settings anytime.
