Identity and Confidentiality in Patient Portal

Authorized representatives (also known as a proxy users) are commonly the caregiver or legal guardian of someone else, for example, a child or elderly parent.

The authorized representative has the authority to carry out health-related tasks on behalf of the patient and manage their care, for example, schedule appointments, view vaccine history, request refills on prescriptions, and more. Authorized representatives may have multiple connected profiles connected to their account.

Authorized representatives generally have the same capabilities as the patient unless information includes any sensitivity restrictions. Depending on the confidentiality and sensitivity restrictions set by your organization and the age of the patient, you might be unable to view some health record information. Providers are responsible for determining whether you can become an authorized representative.

Request Profile Connections

Contact your provider's office if you want to request new profile connections or their removal. If you manage the care of others, for example, if you have children, your provider's office can create additional connections to each profile from your account.

Enroll Patient in Patient Portal

Once your profile connections are established, you can enroll the patient in Patient Portal (if you are not already enrolled). See Activate a New Account and Enroll in the Patient Portal for more information.

Switch Profiles

If you have access to multiple profiles in Patient Portal, for example, your children's, you can access the other profiles by signing in and then selecting the name at the top of the page. After you switch profiles, you can view the other person's health record and manage their care, for example, schedule appointments, view vaccine history, request refills on prescriptions, and more.

Request Removal of Profile Connections

It is the responsibility of each authorized representative to inform their provider immediately of any legal authority changes or when a profile connection needs to be removed.

Contact your provider's office or care team if you want to remove your connection to another profile.

Once a connection is removed, the authorized representative is unable to access the other profile or its information.
