Get Ready (Complete Pre-Visit Tasks) Before a Visit

The get ready process allows you to complete required tasks before you join or arrive to your visit and enter any required personal details needed by the clinic. Completing these details saves you time during arrival (check-in) later.

Note:

The get ready process might be called clipboards, patient intake, pre-check, preregistration, previsits, pre-visit tasks, self-registration, or a similar name, depending on your healthcare organization and settings. Visits might be called appointments or encounters.

Depending on your organization and account settings, you might receive an email or text message alert to remind you when it is time to get ready for an appointment. Additionally, the appointment card on the Today dashboard might include the text Get Ready (or similar message) to alert you to the availability of this process.

Note:

This process is available only during the time period set by your healthcare organization, such as 3 days before the scheduled appointment until 30 minutes before the appointment starts, at which time the arrival (check-in) period starts. For example, if your appointment is scheduled for 12:30 p.m., you can get ready until 12:00 p.m. After 12:00 p.m., you will need to complete the arrival (check-in) process to complete any missing details.
To get ready for your appointment:
  1. Access the patient portal:
    • No account sign-in: If you do not yet have an account but you were given an invitation or a unique link, follow the instructions provided to start the get ready process. You might need to enter your legal first name, legal last name, and date of birth to verify your identity.

      Note:

      Invitations and links expire after a set number of days. If your invitation or link has expired, contact your provider's office or care team to request a new one.
    • Account sign-in: If you already have an account, sign in (log in) to the patient portal. From the Today dashboard select an activity card that includes the text Get Ready. Alternatively, from the Ask Oracle menu Ask Oracle, select Visits, select the upcoming visit to view its details, and then select Get Ready (or similarly named button). The Get Ready (or similarly named) page is displayed.

  2. Complete any details as needed. See the sections below for more information.

    Note:

    Select the Appointment Details tab any time to view the details for the appointment, including its location, phone number, calendar meeting download, required preparations, and post-appointment instructions. You can also select Reschedule if that action is available to you or Add to Favorites (or Remove from Favorites). See Reschedule an Upcoming Visit and Manage and Schedule Favorite Visits for more information about these actions.
  3. When you are ready, select I Am Ready (or similarly named button).
When your appointment is ready for arrival (check-in), you can proceed by selecting Confirm Arrival (or similarly named button) on the Today dashboard or in the appointment details. See Mark Diary Notes for Review for more information.

Complete Questionnaires

Questionnaires (also called clipboards, e-clipboards, registration forms, or surveys) may be assigned to you by your healthcare organization as part of the get-ready process. You might also have consent forms to complete.

Note:

Questionnaires are not intended for diagnostic purposes or calculations. Questionnaires are intended only for patient onboarding, registration (patient intake), and information gathering.
Complete any questionnaires or forms assigned to you by your healthcare organization during the get-ready process:
  1. Select Begin when you are ready to start. A list of required questionnaires is displayed. Depending on you, your healthcare organization, and the type of appointment, different types of questionnaires may be required.
  2. To complete a questionnaire, select the update button (>) to open its form.
  3. Select Update to enter information in a section of the questionnaire. Depending on the questionnaire, multiple sections might be included.
  4. Select Add (or similarly named button) to add new details, or select Edit Edit to edit any existing details. Alternatively, optionally select Remove (or similarly named button) to remove any detail.

    Note:

    If an item that includes a badge of Pending Edit or Pending Add, that update or addition has not yet been reviewed by your provider.
  5. When you are done with a section, select Back (<) next to the page title to return to the list of questionnaires.
  6. Repeat the steps above as needed for each section in each questionnaire.
  7. When you are done completing all questionnaires, select Submit to submit your answers to your provider.

    Note:

    You can only submit questionnaires and forms once. Once you submit them, you will be unable to view or update their information.
If necessary, select Back (<) next to the page title to return to the Get Ready (or similarly named) page.

Review and Edit Personal Details

Enter any missing demographics (personal details):
  1. Select Edit Edit to edit your personal details as needed.
  2. Enter or update any missing details, such as your phone number, address, birth sex, race, and ethnicity, and then select Save to save your changes.

    Note:

    Some details cannot be edited. Contact your provider's office or care team if you need to make updates to any read-only information.

Review and Edit Insurance

Enter your insurance information:
  1. Select Edit Edit to edit your insurance details as needed.
  2. If you do not yet have insurance on file, or you want to add a new insurance provider:
    1. Select Add Insurance to scan your insurance card's bar code or take or upload a photo of your insurance card. Alternatively select I Don't Have One to skip this step. Alternatively, select Enter Manually to enter this information manually. The Add Insurance page is displayed.
    2. Enter any missing details as needed. (If the card's bar code was readable, the insurance information is filled in for you.)
    3. Select Save to save any changes.
    4. Repeat the steps above as needed to add any additional insurance providers. You can have up to three active insurance carriers.
  3. If you already have insurance on file, review your insurance details and make any changes as needed:
    • Select Edit Edit to edit any details.
    • Select Remove (or similarly named button) to remove an insurance carrier.

Review and Edit Identification

Provide your identification card: 
  1. Select Edit Edit to edit your ID details as needed.
  2. If you do not yet have an ID on file:
    1. Select one of the options below. Alternatively, select I Don't Have One or Complete Later to completely skip this step.
      • Driver License
      • State-Issued Identification Card
      • Passport
    2. Use your device to take a photo or upload an image of the ID. Files should be a .JPG or .PNG file with a maximum size of 100 MB. The patient portal uses optical character recognition (OCR) to make a copy of those details.
    3. Select Next to review your address.
    4. Select Update Address to manually update your address or select Do Not Update Address to use the address that was imported from the ID.
  3. If you already have ID on file, review the ID and make any changes as needed:
    • Select Edit Edit to scan a new ID.
    • Select Remove (or similarly named button) to remove a person from your list of contacts.

Review and Edit Emergency Contacts

Enter your emergency contact relationships:
  1. Select Edit Edit to edit your relationships as needed.
  2. If you do not have an emergency contact on file or you want to add a new one:
    1. Select Add Emergency Contact to add emergency contacts. Alternatively, select No Emergency Contacts to skip this step.
    2. Enter the name, relationship, and phone number for the contact.
    3. Optionally, enter the contact's date of birth and address.
    4. Select Save to save any changes.
    5. Repeat the steps above as needed to add any additional emergency contacts.
  3. If you already have emergency contacts listed, review the list of contacts and make any changes as needed.
    • Select EditEdit to make any changes to a person's contact details.
    • Select Remove (or similarly named button to remove a person from your list of contacts.

Review and Edit Responsible Parties

The responsible party (also called guarantor) is the person who is responsible for paying for any health visits. This person might be yourself or an authorized representative.
To review and edit the responsible party:
  1. Select Edit Edit to edit your responsible party as needed.
  2. If you do not yet have a responsible party on file, or you want to add a new one:
    1. Select Add Financially Responsible (or similarly named button).
    2. Add the responsible party's details such as first name, last name, date of birth, relationship to the patient, and phone number.
    3. Optionally, enter additional details such as the title, Social Security number, address, and any other relevant details. The information you enter is always stored securely and safely.
    4. Optionally, select Other Contact Method to add an additional contact method.
    5. Select Save to save any changes.
    6. Repeat the steps above to add additional responsible parties.
  3. If you already have a responsible party listed, review the responsible party's details and make any changes as needed:
    • Select Edit Editto edit a person's details.
    • Select Remove (or similarly named button) to remove a person as a responsible party.
