View Social History

In your health record, the Social History page displays social health history documented by your provider or care team.

To view social history:

1. From the Ask Oracle menu, select Social History. Alternatively, access the My Health dashboard, and then select Social History from the My Health menu.
2. Optionally, select a section to view details.

To make any changes, meet with your provider to discuss your social history.
