You can use the Insert Form wizard to create a form that uses a form handler:
Open a document in the Document editor.
Place the insertion point within the body of the document, and from the Insert menu, select Insert Form.
In the Insert Form Wizard, click the “...” button.
In the Select Form Handler Component dialog box, select [all modules] from the Modules list, then select Form Handlers from the Categories list.
The Components list shows you all of the form handlers available.
Select a form handler and click OK.
The wizard then guides you through the process of creating the form. The dialogs are tailored to the type of form handler you have selected.
