In rare cases, when a USB 1.0/1.1 device is connected to the system (such as a keyboard, mouse or KVM), Oracle Solaris generates a fault.io.usb.not_acc error. In addition, the system service required fault indicator might light.
There is no known functional impact as a result of this issue and the fault can be ignored.
