Patient ID | Identifier for the patient associated with the query. Links to the Patient Summary page. |
Visit Name
| Name of the visit associated with the query.

Form Name | Name of the form associated with the query.
|
Query Status | Status of the query. |
New Value | Current value of the item. |
Old Value | Previously entered value of the item. This field is blank when the query is raised because no data was entered for the item. |
Question Text | Text of the item question on the form. |
Query ID | ID of the query. |
Query Text | Text of the query. |
Back to Query List << Previous, Next >> | Returns to the Query List or navigates to the previous or next query that matches the filters specified on the Query List page. |
Show Filter link | Displays a dialog box that lists the current filter selections on the Query List page. |

Change Data | Question label and data entry fields. |
Site Response Site Response Other Text | Text of the site response to the query. The Site Response Other Text field contains text if the Site Response value is Other. |
Notes History |

Change status | Actions you can select that occur after you click Submit. In the sponsor view, the radio buttons that appear depend on your rights and the current status of the query. In the site view, only the Submit button is active. |

Query ID | ID of the query. |
Date (GMT) | Date that the change action on the query occurred, in GMT. |
Issued By | User who initiated the change to the query. |
Change Event | Type of change action. |
Notes | Text of the query, query response, or comment. |
